What we are looking for:

 

The successful candidate will work collaboratively with the Infections Disease physicians, pharmacy professionals and other members of the interprofessional team to promote the appropriate use of antimicrobial agents at Oak Valley Health.  As a key team member of the Antimicrobial Stewardship Program (ASP), this full time role will be responsible for the development, implementation and ongoing application of antimicrobial policies and procedures (including staff education, participation in multidisciplinary rounds, drug product evaluation for formulary, provision of drug information, promoting appropriate antimicrobial prophylaxis, establishing guidance for empiric antimicrobial therapy, optimizing antimicrobial therapy following a definitive diagnosis, and optimizing antimicrobial dose and route of administration based on pharmacokinetics and pharmacodynamics principles). Participation in committee meetings for initiatives that involve antimicrobials is mandatory. The pharmacist will also work with Information Technology and Decision Support to develop tools to monitor antimicrobial utilization/optimization. The pharmacist will update the First Line app for antimicrobials and report quarterly ASP metrics to the committees.  On a daily basis, this individual is responsible for ensuring hospital-wide compliance with antimicrobial policies and performing prospective and retrospective evaluation of ongoing compliance and providing feedback to pharmacists and physicians regarding appropriate use of antimicrobial agents.  The pharmacist must be able to monitor patients’ drug therapy to detect, resolve and/or prevent drug-related problems and make recommendations for therapy changes, when necessary.. Other duties may be assigned.

 

 

 

What you bring to the role:

 

  • Current A registration status with the Ontario College of Pharmacists
  • Two years of related  experience in a hospital setting required
  • Hospital pharmacy residency or advanced clinical degree with infectious training or equivalent clinical experience is preferred
  • Demonstrated experience with preparing and delivering individual and group education sessions
  • Demonstrated computer skills and familiarity with multiple software programs is essential
  • Strong organizational and time management skills required
    High proficiency in conducting research, compiling information and record maintenance
  • Ability to work independently with minimal supervision
  • Excellent communication and interpersonal skills, and the ability to work collaboratively with the members of the interdisciplinary team
  • Evidence of professional development, continuing education and leadership experience is required
  • Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with organizations related policies, health and safety legislation and best practices and completing relevant mandatory education as required.
  • Demonstrated good attendance and performance records with the ability to maintain these same standards
  • Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required

 

Work Schedule:

The successful candidate will work primarily weekdays and must be available for occasional evenings and weekends, as needed.

 

Compensation:

OPCOM09: $44.70 – 58.78 per hour

$87,165 – 114,621 annually
